If you are a former Colorado Works participant, you may have been contacted by Public Consulting Group, LLC to complete the Colorado Works (Temporary Assistance for Needy Families) Leavers Survey. Public Consulting Group, LLC is conducting this survey on behalf of the Colorado Department of Human Services (CDHS).
About the survey
The Colorado Works Leavers Survey helps CDHS assess the effectiveness of the Colorado Works program and make improvements. The survey is sent to a sample of Colorado Works participants who have recently left the program. The survey includes questions about Colorado Works participants’:
- Experiences in the program
- Beliefs about the goals of the program
- Perceptions on how participation in the program contributed to family goals
- Reasons for leaving the program
- Current employment status and wage rate

Will my answers to the survey affect my benefits?
Your answers will never be used to determine your eligibility for or access to Colorado Works or any other benefits.
How will I get the survey?
A link to the survey may be texted, emailed or mailed to you.
Do I have to participate in the survey?
No, the survey is voluntary, but your participation is important because it helps us assist other families to succeed.
When will I be contacted to take the survey?
Not all former Colorado Works participants will be contacted, as there are too many participants for us to be able to contact every participant. Instead, we select a sample of participants to contact each month. If you are part of the sample, you will be contacted two months after you leave Colorado Works.
Will I receive anything for taking the survey?
You will receive a $20 gift card for completing the survey.
Where’s my gift card?
You should receive your gift card within two weeks of taking the survey and no later than 30 days after taking the survey. If you have a question about your gift card, please contact Christopher Newhard at cnewhard@pcgus.com.
How long does it take to complete the survey?
Most participants complete the survey in around 15 minutes.
