County C-Stat is a system that helps the Colorado Department of Human Services (CDHS) track and improve how well its county-based programs are working. By analyzing data, CDHS can see what’s effective and what needs improvement, allowing for better decisions that lead to positive change. CDHS leadership and staff work together to review data, spot trends and find ways to improve services. Program divisions develop and implement strategies, while leadership helps remove obstacles to their success. This approach focuses on transparency, collaboration and results. By collecting timely data, holding regular meetings, and setting new performance goals, County C-Stat supports ongoing improvements in the services people rely on.

CDHS holds County C-Stat meetings each month to review statewide trends and explore ways to improve county-based services. These meetings — which are led by the CDHS Organizational Performance Management team — bring together state and county staff to discuss data, share best practices and find solutions to challenges.
The meetings last about 90 minutes, with the date and time varying to avoid scheduling conflicts. County directors receive invitations and are encouraged to include relevant staff. When possible, state program offices also invite participants through their mailing lists.
Meetings include presentations from counties that have successfully improved performance in key areas. These presentations provide real-world examples and practical strategies that other counties can use. Attendees receive the agenda in advance to help them prepare questions and discussion points.
For those unable to join live, meetings are recorded and shared afterward. This ensures everyone has access to the information and can apply the insights to their own work.

- Celebrating successes through awards
The Distinguished Performance Award is earned by counties that demonstrate consistent service to their customers. The award celebrates counties that meet their monthly County C-Stat goals at least 75% of the time throughout the year. CDHS visits these counties in June and July each year to celebrate their accomplishment.
